Start here: MODERN MEMBERSHIP ADVANTAGE - SMARTER
This is for the patient who knows they are ready to make changes!
Modern Membership Advantage is a 4 month plan to keep you connected and supported. Each month you will have a total of 2 visits with your doctor.
START by scheduling your FIRST VISIT( INTAKE VISIT) which is 1 hour.
You can pay your membership through the booking site, or contact the office for a monthly payment plan.
Your Report of Findings visit will be scheduled based on labs ordered and is typically an hour long visit. All other visits are 30 minutes each and you can use up to 6 Practitioner visits over the course of the four month membership (max- 2 visits per month, so get them scheduled! They do not roll over from month-to-month.)
You may also schedule a Quick 15 minute Check-in if you have questions. Please ask the office for help scheduling at Team@ModernFunctionalMedicineCenter.com All appointments are VIRTUAL.
FASTER
Current and previous MFMC clients may qualify for the FASTER Membership.
Your progress hits a new level, you're able to resolve issues faster, results come quicker.
This 4 month program includes doctor visits every 3 weeks.
BETTER
Current and previous MFMC clients may qualify for the BETTER Membership.
You're feeling great! You know how to prevent setbacks and stay the course. You are better!
This 4 month program includes 1 visit per month with your doctor.
Well Patient Check-in
Check in 2-appointment FOLLOW UP for previous MFMC Members
This appointment is for our patients that have been through our Modern Membership program, are feeling great, and would like to check in periodically. Your first appointment (#1) will be 45-59 minutes to evaluate and order lab test you need to have ordered. The second appointment (#2 ) will be a Report of Findings.
This is only for patients who are feeling well, and just need a quick check-in.
