Modern Membership Advantage is a 4 month plan to keep you connected and supported. Your will have a visit with your doctor every 21 days.

START by scheduling your FIRST VISIT( INTAKE VISIT) which is 1 hour.
You can pay your membership through the booking site, or contact the office for a monthly payment plan.
Your Report of Findings visit will be scheduled based on labs ordered and is typically an hour long visit. All other visits are 30 minutes each and you can use up to 6 Practitioner visits over the course of the four month membership.

Check in 2-appointment FOLLOW UP for previous MFMC Members

This appointment is for our patients that have been through our Modern Membership program, are feeling great, and would like to check in periodically. Your first appointment (#1) will be 45-59 minutes to evaluate and order lab test you need to have ordered. The second appointment (#2 ) will be a Report of Findings.

This is only for patients who are feeling well, and just need a quick check-in.
